Membership Policies:

Payment due on the 15th of each month.

Your membership is auto renewable following the initial term.

Your membership will automatically continue on a month to month basis until cancel.

You may cancel your membership during the initial term only in the event that you permanently relocate your residence more than 50 miles away from Tina’s Heavenly Touch or a physician certifies that you are unable to receive service purchased.

You may cancel appointments without charge up to 24 hours before your scheduled appointment or a $25 charge will be automatically deducted from the credit card on file.

No Shows will be charged as services received.

To freeze your membership, you must come in and sign documentation.

Any changes to the membership will need to be done 15 days prior to due date to guarantee changes
